puzzle success rate not working.

Has anyone else had the problem that they solve all puzzles but somehow it appears as though I haven't. I have completed all puzzles presented this month. Up till today 30 min ago, my success rate was 100%. Today all of a sudden it says I haven't solved a puzzle, but it is not in my recent puzzle list. So how can I not have solved it?
The site has been a bit glitchy today and required a reload. Is this just a crappy bug that ruins your record.

There has been at least one glitch that has damaged my success rate in the past. At one point, I even figured out how to generate the glitch. Unfortunately, by the time the administrator asked me how, I'd forgotten and couldn't reproduce it. It is something that doesn't show up in your incomplete puzzle list, so it can't be recovered from...
