Question Completion percentage and awards

I can only assume this will be corrected at the end of May - there are three users listed with greater than 100% completion percentage - or do they know something the rest of us don't?? Also found...

Just another person requesting Undo feature and displaying Clues first when puzzle is loaded. For Undo, a button that Saves the current state of the puzzle would be required - Undo would then go to...

What constitutes failed puzzle

What determines a failed puzzle? Requesting a hint? Submitting an incorrect solution? Something else? Thanks - I'm new to site but have enjoyed logic puzzles since I was a kid.
